I want to clear out a couple files that were doubled by a uncontrolled file sync (by OneDrive)
Yet when I filter by “- Copy” I always see one file without the search text. Is this by design or a flaw in Nautilus?

You may want to ask this question on discourse.gnome.org
Your search might also include files indexed with GNOME Local Search (formerly known as Tracker Indexer).
In order to limit your search to file names only, you would need to press the Filter Search Results button (in shape of two sliding buttons), then in What field select Files.
